Just as insulin pumps and continuous glucose monitors have improved how people manage their condition, digital tools are now helping your clients take control of their health information and stay connected with their care team.

Managing diabetes requires regular monitoring, ongoing treatment and coordinated care. By supporting your clients to actively engage with their health data, you can help improve outcomes and empower them to live well with the condition.

The my health app, developed by the Australian Digital Health Agency and powered by My Health Record, is one such tool making a difference.

Help your clients access their health information

The my health app is a secure and convenient way for your clients to view key health information in their My Health Record and can also store electronic prescriptions and more. It’s fast, easy and secure – and they can access it all from the palm of their hand.

Features that support diabetes management

Access to My Health Record
In my health app, your clients can view blood glucose results, HbA1c levels and other clinical data uploaded by you or other members of their care team. This helps to monitor trends, stay informed and work with you to adjust treatment plans when needed.

Electronic Prescriptions
The my health app allows clients to receive and store electronic prescriptions sent via SMS or email. This is especially helpful for those who need regular medication or live in rural or remote areas.

You can also encourage your clients to register for an Active Script List (ASL) at their pharmacy. An ASL is a digital list of all active electronic prescriptions, which the pharmacy can access to simplify dispensing.

Pathology and Imaging Results
Your clients can view pathology and diagnostic imaging results and reports in my health app. Work is currently underway which aims to make most results available as soon as they are uploaded. Some results, such as HbA1c, are already accessible as soon as they are uploaded. This supports timely follow-up and helps your clients stay engaged in their care.
